Q. What role should physicians play in case management population health efforts?
A. The physician is prominent in acute hospital care as well as primary care and may be viewed as the team lead. Although the physician drives the medical component of a patient’s recovery, he or she still requires the entire team to advance the patient’s personal goals and to link them with the team’s healthcare goals. The success of the integrated case management model requires the physician to maintain an open mind to input from first the patient and then all of the other team members.
Physicians working in the population health model will need to tailor their practice to the team approach and appreciate the contributions of the other healthcare team members to mitigating social, behavioral, and functional risks to patients.
