Clinic Aims to Improve PCP, Patient Relationships Through Case Management

A clinic in Stafford, Arizona, is expanding its services and focus on case management in an effort to improve relationships among primary care providers and patients, according to The Graham Star.

This is just one of the many changes underway at Hilltop Health Care, a free clinic operated by Arizona’s Graham County Department of Public Health. It is also working toward improving patient health literacy, another goal that may be possible with enhanced case management. With a new grant cycle approaching, the clinic analyzed its resources and identified a need for additional case management care.
