The benefits of evidence-based patient care

A one-size-fits-all approach to case management and patient care can be detrimental to patients and the case management profession, but so too can designing a new approach to care plans for every patient who walks through the door, according to an article in CMSA Today.

Instead, case managers should consider evidence-based care plans when treating complex patients and allow socioeconomic factors, comorbidities, and demographics to drive care. Developing care plans based on evidence-based practices means that case managers can have a common ground from which to begin their plan of care using standardized assessments. It also ensures that a patient’s individual progress and goals can be measured against standardized benchmarks, according to CMSA Today.

While some physicians have a wealth of evidence-based data at their fingertips based on their specialty, the same is not always true when it comes to support for one case management initiative over another, according to CMSA Today. Embracing the idea that standardized care plans and evidence-based practices can still provide patients with personalized care and help the case management profession progress is an important step.
