Q&A: Reporting hemodialysis and CKD in ICD-10-CM with a failed transplant
Q: In ICD-10-CM, how would you report a patient who is receiving hemodialysis and has chronic kidney disease (CKD) when a failed kidney transplant is also documented?
A: You first need to make sure that the kidney the patient is receiving dialysis for is the kidney that they received the transplant for. There is a possibility that the old kidney could still remain, but typically the patient is relying on the transplanted kidney.
Based on the information given, the renal transplant should be coded as a complication. ICD-10-CM code T86.12 (kidney transplant failure) would be assigned along with a CKD code from category N18.- (chronic kidney disease). You should also report the hemodialysis status along with your hemodialysis procedure code.
